What tests are prescribed before IVF

Before recommending IVF, the reproductive specialist assigns a range of tests to both partners to determine the cause of infertility. As a rule, this diagnosis is made in the absence of pregnancy for 12 months. After identifying the cause, the doctor may prescribe infertility treatment or recommend the use of ART methods.

Hormonal tests (for both women and men)

Women are tested for FSH, LH, estradiol, prolactin, AMH, and TSH to assess ovarian reserve, ovulation regularity, and endocrine system function. In men, testosterone, prolactin, and gonadotropins are tested to detect spermatogenesis disorders. Blood is drawn on an empty stomach, and the results help the doctor create a personalized treatment plan.

Pelvic ultrasound (for women)

A transvaginal ultrasound evaluates the structure of the uterus, endometrial thickness and condition, the anatomy of the ovaries, and the number of antral follicles. This is an essential test for ovulation stimulation planning and ruling out factors that may interfere with embryo implantation. It is usually performed on days 2–4 of the menstrual cycle.

Semen analysis (for men)

This test of ejaculate allows assessment of sperm count, motility, and morphology. The results help evaluate male fertility and determine whether to use traditional IVF or ICSI. Before the test, sexual abstinence for 3–5 days is required, and stress or overheating should be avoided.

TORCH-complex, HIV, hepatitis B and C, syphilis (for both partners)

These tests are mandatory for both partners before starting the IVF program. They help detect hidden or active infections that could threaten the future pregnancy or fetus. If infections are found, appropriate treatment is carried out before starting IVF. Tests are done using venous blood.

Karyotyping (for both partners)

This is a genetic test of chromosome structure and number. It helps detect chromosomal abnormalities that may cause infertility, recurrent miscarriages, or failed IVF attempts. It is prescribed for couples with unsuccessful pregnancies, hereditary diseases, or failed ART cycles.

Hysteroscopy (for women)

An endoscopic examination of the uterine cavity that helps detect anatomical changes (polyps, adhesions, septa, fibroids) that may hinder embryo implantation. Performed in the second phase of the cycle under mild anesthesia. If necessary, the procedure may also be therapeutic: pathological formations can be removed during the procedure.

How to prepare for IVF

What tests are mandatory before starting the IVF program?

Before treatment begins, the couple undergoes a full medical examination. The woman receives an ultrasound, hormonal tests, infection screening, and an assessment of ovarian reserve. The man provides a semen analysis and additional tests if needed. The scope of diagnostics is determined by the doctor depending on the couple’s history and age.

Should chronic diseases be treated before IVF?

Yes. Before the program, all chronic conditions (hypertension, diabetes, thyroid disorders, etc.) must be stabilized. It's also important to treat urogenital infections and perform oral cavity sanitation. This improves the chances of success and reduces the risk of complications.

Should lifestyle changes be made before IVF?

It is recommended to quit smoking and alcohol, reduce physical and emotional stress. It is also important to normalize weight, establish a regular sleep schedule, healthy diet, and drinking regimen. A balanced lifestyle improves stimulation effectiveness and embryo quality.

Stages of In Vitro Fertilization

The IVF program consists of several distinct stages. Each stage is overseen by a physician and embryologist, and the plan is adjusted based on the individual characteristics of the couple.

Ovulation stimulation

The woman is prescribed hormonal medications that stimulate the growth of multiple follicles instead of one. This allows for the retrieval of several eggs and increases the chances of successful fertilization.

Egg retrieval

Under ultrasound guidance and anesthesia, the ovaries are punctured to collect follicles. The retrieved material is sent to the lab to isolate the eggs.

Fertilization and embryo culture

Eggs are fertilized with sperm (partner’s or donor’s), either traditionally or via ICSI. Embryos are cultured in a special environment for 3–5 days.

Embryo transfer to the uterus

The highest-quality embryos are transferred into the uterine cavity using a flexible catheter under ultrasound control. The procedure is painless and performed on an outpatient basis.

Pregnancy confirmation

10–14 days after transfer, a blood test for hCG is done, followed by a control ultrasound to confirm and assess pregnancy development.

Contraindications to IVF

Unfortunately, IVF is not suitable for everyone. There are absolute and relative contraindications (in the latter case, the procedure may be postponed).

Absolute contraindications include mental illness, malignant tumors, HIV/AIDS, dysfunction of internal organs, uterine underdevelopment or damage that prevents carrying a pregnancy.

Relative contraindications include acute illnesses, exacerbation of chronic diseases, benign tumors, physiological ovarian exhaustion, and some other conditions. These are usually resolved with treatment, after which IVF can proceed.
