Adrenal gland cancer is a malignant disease characterized by the development of a neoplasm in one or both organs at once. It can be primary - the tumor originates from the cells of the brain or epithelial layer of the adrenal gland, or secondary - a metastasis of cancer of another location, in particular the lungs, stomach, pancreas, colon, kidneys, melanoma or lymphoma.

Metastatic damage to the adrenal glands occurs much more often than primary cancer. Its prevalence is 1-2 cases per 1 million people. At this time, benign tumors, on the contrary, are common - they are diagnosed in 5% of men and women. Usually they do not disrupt the functioning of the glands and are detected by chance during diagnosis for another reason. Histological analysis allows you to distinguish them from malignant ones.

With early diagnosis, adrenal gland cancer is successfully treated. If it is a metastasis, the prognosis depends on the underlying disease.

Stages of adrenal cancer

There are 4 main stages of adrenal cancer:

  • Stage 1 – the size of the neoplasm is less than 5 cm, it is localized in the organ, the lymph nodes are not damaged;
  • Stage 2 – the size of the tumor is more than 5 cm, but it does not go beyond the adrenal glands;
  • Stage 3 – the size of the tumor is more than 5 cm, it grows into neighboring organs, affects the lymph nodes;
  • Stage 4 – the tumor spreads to neighboring organs and gives distant metastases.

This division is relative. To determine the stage of cancer, oncologists use the international TNM classification system, which involves a separate assessment of three parameters: tumor size, lymph node involvement, and the presence and number of metastases.

Risk factors and prevention of adrenal cancer

According to studies, in 15% of cases, the cause of adrenal cancer is genetic disorders. In other patients, the disease develops as a metastasis or as a result of cell mutation triggered by risk factors.

These include:

  • hereditary predisposition;
  • congenital anomalies of the adrenal glands;
  • injuries in the lumbar region;
  • operations on the adrenal glands;
  • diseases of the endocrine system;
  • immune disorders;
  • hypertension;
  • increased excitability of the nervous system;
  • smoking;
  • alcohol abuse;
  • unbalanced diet;
  • interaction with toxic substances;
  • unfavorable environmental conditions.

For prevention, it is recommended to lead a healthy lifestyle, regularly undergo preventive examinations and timely treat all diseases.

Symptoms of adrenal cancer

Small tumors develop in a latent form. Symptoms of the disease appear when they increase
and put pressure on neighboring organs, general intoxication and sometimes excessive production
of hormones. As a rule, the symptoms are nonspecific and do not allow to understand
the cause of the malaise without comprehensive diagnostics.

In children, a change in the level of sex hormones is also possible and, as a result, early development of sexual
signs or the appearance of excess hair in girls and the growth of the mammary glands in boys.

Adrenal cancer may be accompanied by:

menstrual cycle disorders and signs of masculinization in women

lower back or abdominal pain

frequent urination

muscle spasms

increased blood pressure

increased blood sugar levels

weight gain

the appearance of signs of feminization in men

subfebrile temperature

loss of appetite

apathy, depression

weakness

Diagnosis of adrenal cancer

To diagnose the disease, the patient may be prescribed:

  • US ultrasound of the adrenal glands;
  • CT or MRI of the adrenal glands with contrast;
  • blood tests for hormones (testosterone, androstenedione, cortisol, ACTH, DHEA-sulfate, 17-OPH and others);
  • biopsy of the neoplasm - taking a tissue sample by puncturing the adrenal gland with a thin needle;
  • histological analysis - assessing the morphological features of the cells to determine whether they are benign or malignant.

Patients are also recommended to have a CT scan of the brain, chest, abdomen and pelvis to rule out the presence of other tumors.

Adrenal cancer treatment

For the treatment of adrenal cancer, complex therapy is necessary. If possible, first of all, an operation is performed to remove the tumor. Depending on its location and the number of foci, a classic or laparoscopic approach is used - this is a minimally invasive intervention, which is performed using special equipment that is inserted into the abdominal cavity through 3 punctures, up to 1 cm in diameter.

Since the adrenal gland is small in size, it is removed completely. If both glands are affected and/or the cancer has spread to adjacent organs, the volume of the operation increases accordingly.

How is adrenal cancer detected?

Adrenal cancer in the early stages develops asymptomatically. Signs of the disease occur as the tumor grows and are nonspecific. The patient may be bothered by back and abdominal pain, frequent urination, weakness, hormonal disorders, low fever, etc. To clarify the exact diagnosis, a comprehensive examination is required, since these symptoms are characteristic of many diseases, and adrenal tumors often manifest as metastases.

How quickly does adrenal cancer develop?

Adrenal gland tumors can develop at different rates. Some patients may take several years to develop symptoms, while others may take much less. It all depends on the individual. Once the lymph nodes are affected, the disease progresses more quickly.

What is the life expectancy with adrenal cancer?

When diagnosed at stage 1, more than 80% of patients overcome the 5-year survival threshold. At stage 2, this figure is 60%, at stage 3 - about 50%, and at stage 4 - less than 20%.
