Cancer of the appendix is ​​a malignant disease that develops in the initial part of the large intestine. The appendix is ​​located between the ileum and the colon. It has the shape of a dome and performs the functions of absorbing water and nutrients, as well as the initial formation of fecal masses.

Tumors of the appendix account for 20% of all cases of colorectal cancer. Due to the absence of pronounced symptoms in most patients, they are diagnosed at a late stage. Depending on the degree of lymph node involvement

  • N0 – lymph nodes are not affected;
  • N1 – 1 to 3 regional lymph nodes are affected;
  • N2 – more than 4 lymph nodes are affected, including distant ones.

By the presence and number of metastases

  • M0 – no metastases;
  • M1 – metastases in distant organs.

Most patients find it difficult to understand this system, so a simplified one is most often used.

Simplified system

It distinguishes 4 stages of cecal cancer:

  • Stage 1 – the tumor develops in the tissue of the intestinal lining;
  • Stage 2 – the tumor grows into the intestinal cavity;
  • Stage 3 – the tumor extends beyond the cecum, affects nearby lymph nodes;
  • Stage 4 – the tumor spreads to neighboring organs, has distant metastases.

Symptoms of appendix cancer

Symptoms of colorectal cancer in the early stages are almost absent. They appear with
an increase in the size of the tumor, intoxication of the body and damage to other organs.
In case of tumors of the cecum, timely diagnosis is complicated by the fact that patients do not have changes in stool and problems with defecation, which are characteristic of other types of intestinal cancer. The complaints that appear are characteristic of many diseases, including ulcerative colitis, Crohn's disease and others.

It is necessary to undergo a comprehensive examination if the following symptoms appear:

pain in the lower right abdomen

change in stool color (may turn black due to bleeding)

increased gas formation

unexplained weight loss (more than 5% of body weight in 6-12 months)

general weakness, fatigue

subfebrile temperature for a long time

iron deficiency anemia

change in liver size

Diagnosis of cecal cancer

During the first consultation, the doctor will listen to the complaints and carefully study the anamnesis, as well as conduct an examination. After that, he will prescribe tests and instrumental examinations that will help to clarify the diagnosis.

These may be:

  • general blood, urine and stool tests;
  • stool analysis for occult blood;
  • blood tests for tumor markers;
  • transrectal ultrasound of the rectum;
  • colonoscopy - endoscopic examination of the colon;
  • irrigoscopy - X-ray of the intestine with the use of a contrast agent;
  • biopsy - taking a sample of tumor tissue (performed during colonoscopy);
  • histological analysis - determining the type of cells and their malignancy;
  • immunohistochemical analysis - assessing the characteristics of a malignant tumor;
  • ultrasound, CT or MRI of different areas.

An accurate diagnosis can be made by histological analysis of a tissue sample. To obtain a biopsy during a colonoscopy, a small piece of the tumor is pinched off with surgical microinstruments.

A colonoscopy is an endoscopic examination that involves inserting a flexible catheter equipped with a video camera into the rectum. The image from it is magnified and transmitted to a monitor screen, allowing the doctor to examine the mucous membrane of the organ in detail.

Other examinations help clarify the diagnosis and determine how far the cancer has spread.

Treatment of cecal cancer

In case of cancer of the appendix, complex therapy is performed. For each patient, doctors draw up an individual treatment program taking into account the stage of the disease and the general state of health.

It may include a different combination of the following methods:

  • surgical treatment - removal of the tumor;
  • chemotherapy;
  • radiation therapy;
  • targeted therapy;
  • immunotherapy;
  • symptomatic therapy to relieve cancer symptoms and side effects of treatment.

If the cancer is diagnosed at an early stage, surgery is performed first. In some cases, with radical removal, this is enough to defeat the disease. If there is a risk of recurrence, the patient may be prescribed chemotherapy or other treatment.

If a tumor is detected at stage 3-4, chemotherapy or radiation therapy may be required first. They allow you to slow down the growth of the tumor and reduce it. After surgery, treatment courses are usually repeated.

Risk factors for appendix cancer and prevention methods

The risk of developing cancer of the appendix increases with:

  • polyps and benign neoplasms of the intestine;
  • chronic inflammatory bowel diseases (ulcerative colitis, Crohn's disease);
  • genetic predisposition and mutation of the hMLH1, hMSH2, hMSH6, hPMS1 and hPMS2 genes;
  • history of cancer;
  • radiotherapy of the abdominal organs;
  • diabetes mellitus;
  • obesity;
  • smoking;
  • alcohol abuse;
  • unbalanced diet.

The probability of developing cancer of the appendix in people without risk factors who lead a healthy lifestyle is from 1% to 3%. In the presence of risk factors, it increases many times. In particular, with a hereditary predisposition (development of cancer in close relatives), the risk increases to 30%, with large polyps - to 18-56% (depending on the number and size), and with Crohn's disease and ulcerative colitis, which have been going on for more than 20 years, to 15 and 50%, respectively.
