CT scan of the lumbar spine

CT scan of the lumbar-sacral spine in Kyiv

Computed tomography of the lumbar spine — is an informative method of radiological diagnostics that allows visualization of bone structures, intervertebral discs, ligaments and surrounding tissues in the lumbar and sacral spine. Due to its high resolution, CT helps to detect a wide range of pathologies — from degenerative changes to tumor processes. The examination is fast, painless and can be performed with or without contrast, depending on the clinical situation.

At the medical center «Oxford Medical» CT is performed on a modern 64-slice Siemens Somatom Go.UP tomograph, which provides high quality images with minimal radiation exposure.

Indications for CT of the
lumbar spine

Computer tomography is shown to patients in case of suspicion
for structural disorders or pathological changes in the lumbar area
or lumbosacral spine. Research helps to confirm
or exclude the diagnosis, assess the degree of damage and plan treatment.

Main indications:

Persistent or intense lower back pain that does not go away after rest.

Limited mobility, stiffness in the lumbar region.

Suspicion of intervertebral hernia, protrusions or stenosis of the spinal canal.

Post-traumatic conditions - suspected fracture or displacement of the vertebrae.

Planning a surgical intervention or evaluating the results of an operation.

Symptoms of innervation disorders: numbness, weakness, decreased sensitivity in the legs.

Lumbar Spine CT With and Without Contrast

CT without contrast is the basic form of the examination, allowing assessment of the condition of bone structures, intervertebral discs, detection of hernias, osteophytes, fractures, protrusions, and narrowing of the spinal canal.

CT with contrast is performed according to clinical indications when it is necessary to clarify the nature of vascular changes, determine the presence and extent of tumor or inflammatory processes, as well as assess the structure and function of certain tissues. The contrast agent is injected intravenously and accumulates in tissues depending on their vascularization, which makes it possible to clearly visualize pathologically altered areas during tomography.

Preparation for Lumbar-Sacral Spine CT

CT of the lumbar or lumbosacral spine without contrast does not require special preparation. The patient can undergo the examination at any convenient time without restrictions in diet or medication.

Preparation is needed only for CT with contrast. In this case, it is recommended to:

  • Avoid food for 4 hours before the examination.

  • Drink enough water during the day before the procedure.

  • Inform the doctor about allergies to iodine-containing drugs or contrast agents.

  • Report any chronic diseases (especially of the kidneys, liver, thyroid gland).

  • If a kidney function test (creatinine level) is prescribed, complete it in advance.

  • Wear clothing without metallic elements (zippers, buttons, accessories).

How the Lumbar-Sacral Spine CT Procedure Is Performed

The procedure takes 5 to 15 minutes and is completely painless. The patient lies on a movable scanner table, which automatically slides into the ring-shaped part of the device. During scanning, it is important to remain still to obtain clear images.

Stages of the procedure:

  • Preparation and patient instructions.

  • Placement on the CT table in a supine position.

  • Contrast injection if needed.

  • Scanning (takes a few minutes).

  • Completion of the procedure and image processing.

In case of CT with contrast, the patient may feel short-term warmth or a metallic taste in the mouth after injection — this is a normal reaction. After the procedure, patients are advised to drink more water to accelerate contrast elimination from the body.

Interpretation of Lumbar Spine CT Results

The obtained images are analyzed by a radiologist, who describes the detected changes. The report includes:

  • Condition of vertebral bodies, presence of fractures or degenerative changes.

  • Size and location of intervertebral hernias and protrusions.

  • Signs of spinal canal stenosis.

  • Presence of tumors, cysts, osteophytes.

  • Features of joint and sacroiliac articulation structures.

  • Changes in soft tissue structures within the examination area.

The patient should consult the doctor who ordered the CT. Based on medical history, symptoms, previous tests, and the radiologist’s report, the doctor establishes the final diagnosis and determines further treatment strategy.

Q&A

What symptoms indicate the need for lumbar spine CT?

Indications include chronic or acute lower back pain, limited mobility, numbness or weakness in the legs, suspected hernias, protrusions, fractures, tumors, or other spinal structure changes. The examination is prescribed by a doctor after evaluating symptoms and medical history.

Can CT be done during pregnancy?

CT is not recommended during pregnancy, especially in the first trimester, due to the effect of ionizing radiation on the fetus. In exceptional cases, when the benefits outweigh the risks and no alternatives are available, the examination may be performed with a doctor’s referral and safety precautions.

What is the difference between CT and MRI of the lumbar spine?

CT provides better visualization of bone structures, fractures, and calcifications, while MRI is more informative for assessing soft tissues, discs, ligaments, and nerve structures. The choice of method depends on the clinical case and expected diagnostic information.
