Chromoendoscopy — is an additional method of endoscopic diagnostics that is used during gastroscopy or colonoscopy for a more detailed assessment of the mucous membrane. It helps to more accurately assess small neoplasms, their structure and boundaries, as well as detect changes in the mucosa whose nature is difficult to determine during a standard endoscopic examination.

At the «Oxford Medical» medical center, endoscopic examinations are performed using the Olympus Exera II video system with NBI technology. Narrow-band imaging enhances the contrast of the vascular pattern and mucosal structure, which helps to assess detected changes in greater detail. To undergo the examination, make an appointment for a consultation with an endoscopist.

What GI tract chromoscopy shows

Chromoendoscopy is used to clarify changes in the mucous membrane of the esophagus, stomach, and large intestine detected during an endoscopic examination. It makes it possible to assess their structure, surface, and boundaries in greater detail.

Chromoendoscopy of the stomach and esophagus makes it possible to better assess erosions and ulcers, atrophic changes in the mucosa, areas of dysplasia, polyps, and tumor formations. If a certain area differs in structure from the surrounding tissues, the doctor can more accurately determine the site for a targeted biopsy. Then the tissue sample is sent for histological examination to exclude GI tract cancer or precancerous changes.

During intestinal chromoscopy, polyps, dysplasia, and other neoplasms can be assessed more accurately. The examination is also important in long-term inflammatory bowel diseases, particularly ulcerative colitis and Crohn’s disease, when it is necessary to detect precancerous changes in a timely manner.

Indications for chromoendoscopy

Chromoendoscopy of the stomach or intestine is performed as an additional stage of gastroscopy
or colonoscopy if a standard examination is insufficient for an accurate assessment
of the detected changes.

Main indications:

detection of polyps and other neoplasms;

suspected dysplasia or precancerous changes;

the need for a targeted biopsy;

clarification of the boundaries of the formation before its removal.

List of diseases

Chromoendoscopy is also used to monitor the condition of the mucosa in diseases that increase the risk of precancerous and malignant changes. These include gastritis, Barrett’s esophagus, atrophic changes of the stomach, long-term ulcerative colitis, and Crohn’s disease with involvement of the large intestine. In such cases, the examination helps to detect dysplasia in a timely manner and determine further management.

Contraindications to chromoendoscopy

Since chromoendoscopy is performed during gastroscopy or colonoscopy, the main contraindications are related to the possibility of safely performing the endoscopic examination itself.

The procedure may be postponed in case of:

  • severe general condition or decompensation of cardiovascular or respiratory diseases;

  • suspected perforation of the organs of the gastrointestinal tract;

  • acute peritonitis;

  • pronounced narrowing of the intestinal lumen that does not allow the endoscope to be passed safely.

If a special dye is planned to be used during chromoendoscopy, the doctor takes into account possible contraindications specifically to the use of this substance. Before the examination, you need to inform the doctor about known allergic reactions, chronic diseases, and medications that you take regularly.

How to prepare for chromoendoscopy

Preparation for chromoendoscopy depends on which examination it will be performed during — gastroscopy or colonoscopy.

If chromoendoscopy is performed during gastroscopy, it is necessary to:

  • not eat for at least 6 hours before the examination;

  • not drink water for 2 hours before the procedure;

  • not smoke or chew gum on the day of the examination.

If chromoendoscopy is performed during colonoscopy, it is necessary to:

  • exclude fresh vegetables and fruits, greens, berries, seeds, nuts, and whole-grain products 2–3 days beforehand;

  • cleanse the intestine using a laxative according to the regimen that the doctor provides when prescribing the examination.

If gastroscopy or colonoscopy will be performed with sedation, you cannot drive a car after the procedure, so it is worth planning in advance how you will be able to return home.

Doctors at «Oxford Medical» recommend strictly following the preparation rules, especially before colonoscopy. Proper bowel cleansing is necessary for a complete examination of the mucous membrane.

How chromoendoscopy is performed

Chromoendoscopy is performed during gastroscopy or colonoscopy. First, the doctor performs a standard examination of the mucous membrane, after which, if necessary, proceeds to chromoendoscopy.

In classic chromoendoscopy, a special dye is applied to the mucosa through the endoscope. After it is distributed, the doctor re-examines the required area and assesses it under magnification on the screen.

Virtual chromoscopy is performed without applying dyes. Special endoscopic imaging modes are used for it, which enhance the contrast of the vascular pattern and the structure of the mucous membrane.

If a polyp, an area of dysplasia, an ulcer with atypical changes, or another formation is detected during the examination, the doctor may perform a targeted biopsy. A small tissue fragment is taken through the instrumental channel of the endoscope and sent for histological examination, which makes it possible to determine the nature of cellular changes and confirm or exclude a malignant process.

Recommendations after chromoendoscopy

After chromoendoscopy, the recommendations depend on whether it was performed during gastroscopy or colonoscopy, as well as on the use of sedation.

Main recommendations after the examination:

  • if chromoendoscopy was performed during gastroscopy with local anesthesia of the throat, do not eat or drink until sensitivity is fully restored;

  • after colonoscopy, drink enough water and gradually return to your usual diet;

  • in case of bloating after colonoscopy, move more — calm walking helps gas pass;

  • if sedation was used, for 24 hours do not drive a car, consume alcohol, or perform work that requires a quick reaction and concentration of attention.

Doctors at «Oxford Medical» explain: if the histological examination confirmed a malignant neoplasm, the patient may be recommended an oncologist consultation. Further examination and treatment are determined taking into account the histology results and additional diagnostics.

After endoscopy, it is important to monitor your well-being. In case of severe or increasing pain, significant bleeding, increased temperature, repeated vomiting, or a sharp deterioration in condition, it is necessary to consult a doctor.

Frequently asked questions

How does chromoendoscopy differ from conventional endoscopy?

During standard gastroscopy or colonoscopy, the mucous membrane is examined in white light. Chromoendoscopy additionally enhances contrast using a dye or a special imaging mode, which helps to better assess small formations, the structure of the mucosa, and the boundaries of the detected changes.

Are the dyes used during chromoscopy safe?

The dyes used for chromoendoscopy are usually well tolerated. Before the procedure, the doctor clarifies the presence of allergic reactions and takes into account possible contraindications to a specific substance. In virtual chromoscopy, dyes are not used — the image of the mucosa is enhanced using a special mode of the endoscopic system.

How long does chromoendoscopy take?

Gastroscopy usually lasts about 15–30 minutes, while colonoscopy lasts 30–45 minutes. Performing chromoendoscopy may increase the duration of the procedure by approximately 10–20 minutes depending on the area of the mucosa that needs to be examined and the need to perform a biopsy.
