Most often, a testicular biopsy is performed using one of these methods:
Aspiration (vacuum) collection of sperm from the testicle. It is performed using a thin needle, which is inserted into the testicle through a puncture of the skin of the scrotum. TESA is also called puncture, because it does not require an incision and is the least traumatic. It is most often performed under local anesthesia.
A surgical removal of a slightly larger volume of testicular tissue, which is performed through an incision in the skin of the scrotum and testicle. The TESE procedure takes about 30 minutes and is performed under sedation.

As part of preparation for the biopsy, laboratory tests of blood and urine are performed.
