It consists in fixing the organ in an anatomically correct position. Previously, such operations were performed only through open access, but now laparoscopy has become the “gold standard”. It allows you to perform the necessary manipulations through several punctures with a diameter of about 1 cm, which minimizes damage to healthy tissues, reduces the risk of complications, and the rehabilitation period is faster and easier.

When is laparoscopic nephropexy prescribed?

Laparoscopic nephropexy is prescribed to patients with nephroptosis, or wandering kidney syndrome of stages 2 and 3, when the displacement of the organ relative to the anatomical localization reaches 5 centimeters or more. In this case, the disease leads to tension of the neurovascular bundle.

Asymptomatic nephroptosis of the 1st degree (kidney deviation less than 5 cm) usually does not require surgical treatment. Patients are prescribed dispensary observation - an examination by a urologist with urine tests and ultrasound of the kidneys every 6 or 12 months.

lower back pain that worsens when standing, walking, and exercising

Preparing for surgery

Before the operation, the patient undergoes a comprehensive examination to confirm the presence of indications and exclude contraindications.

Nephropexy is only required for nephroptosis of the 2nd and 3rd degree, when the deviation of the kidney in the standing and lying position is more than 5 cm, and the patient also has symptoms of the disease.

Diagnosis may include:

  • examination by a nephrologist;

  • ultrasound of the kidneys and urinary tract;

  • urography (X-ray of the kidney with contrast);

  • urine and blood tests;

  • consultation of an anesthesiologist or a doctor of another specialty if the patient has a chronic disease.

Preparation for laparoscopic nephropexy may require preliminary bowel cleansing with an enema. The patient is recommended to follow a light diet for a few days, and on the day of the operation to come to the clinic on an empty stomach. Regarding taking any medications before the operation, it is necessary to consult a doctor in advance. 

Treatment of kidney prolapse

How is laparoscopic nephropexy performed?

Before the operation, the patient is put into a state of anesthesia. After that, the surgeon makes several small punctures on the abdominal wall, through which he inserts trocars - surgical instruments in the form of tubes, about 1 cm in diameter. One of them is equipped with a video camera with a built-in LED, the image from which is transmitted to the monitor screen and magnified many times, and the others - with miniature surgical instruments. To ensure a high-quality examination and space for manipulations, carbon dioxide is pumped into the abdominal cavity, which provides a lift of the abdominal wall.

Next, the surgeon conducts a laparoscopic examination of the kidney, determines the degree of its deviation and chooses the optimal fixation site. After that, he moves the kidney to a physiological position and installs a mesh ribbon endoprosthesis. It is fixed to the fascia, quadratus lumborum muscle or other tissues.

The endoprosthesis is made of biocompatible material (polypropylene), which successfully takes root and is not rejected by the body. It is not felt by the patient after the operation and does not affect the blood supply to the kidney and the normal outflow of urine. Thus, the operation allows to restore the physiological position of the kidney, after which the symptoms of the disease in most patients disappear.

After the operation, the surgeon conducts a thorough examination of the abdominal cavity, if necessary, coagulates the bleeding vessels, after which he pumps out the carbon dioxide, removes the trocars and applies small stitches at the puncture sites.

Rehabilitation after laparoscopic nephropexy

After the operation, the patient is transferred to the intensive care unit, where he remains under hardware monitoring and the doctor's supervision until he fully recovers from anesthesia. After a few hours, if all vital signs are normal, the patient is transferred to the hospital.

Gradually, the diet is expanded, but until recovery, you cannot eat salty, spicy, smoked, fried foods, as well as drink alcohol. You should choose steamed or boiled dishes.

For several weeks, the patient is recommended to wear a bandage, avoid heavy lifting and heavy physical exertion. In most patients, the rehabilitation period is about 1 month, in rare cases - up to 3 months.

Potential complications of kidney prolapse

In nephroptosis of the 2nd and 3rd degree, patients are recommended surgical treatment. Conservative methods, in particular, constant wearing of a bandage, are ineffective in such cases.

Refusal of surgery or untimely consultation with a doctor can lead to serious complications:

  • pyelonephritis (inflammation of the kidneys);

  • infectious diseases of the genitourinary system;

  • urolithiasis;

  • arterial hypertension.
