Gastric resection – is an operation to remove a part of the organ affected by a tumor or other disease. During its implementation, an anastomosis is also formed: the undamaged part of the stomach is connected to the jejunum or duodenum. When is gastric resection prescribed?

Indications for gastric resection are:

  • malignant and benign neoplasms;

  • chronic peptic ulcer disease in case of ineffectiveness of conservative therapy and high risk of complications;

  • perforation of the stomach wall;

  • hypertrophic pyloric stenosis;

  • Zollinger-Ellison syndrome.

Preparation for gastric resection

To determine the size and location of the tumor or assess other pathological changes before surgery, the following are performed:

  • gastroscopy with simultaneous biopsy for histological analysis of a tissue sample;

  • stomach radiography with contrast enhancement;

  • CT of the abdominal organs and other examinations as indicated.

Immediately before surgery, in order to exclude contraindications to anesthesia and high risks of complications, general and biochemical blood and urine tests, blood tests for group and Rh factor, for coagulation, for hepatitis B, C, HIV and syphilis, as well as electrocardiography (ECG) are also prescribed.

2-3 days before hospitalization, the patient is recommended special diet: it is allowed to eat light meals, semi-liquid or liquid consistency.

How is a gastric resection performed?

Gastric resection is performed under general anesthesia. Thanks to this, the patient does not feel pain and wakes up after the operation.

During the operation, the surgeon removes the affected stomach tissue, after which an anastomosis is formed.

After the operation, the patient is brought out of anesthesia. He is left in the postoperative ward for several hours under hardware monitoring, after which he is transferred to a regular ward of a surgical hospital.

Depending on the location and size of the tumor, the following are used:

Laparoscopic access

Three punctures, up to 1 cm in diameter, are made in the abdominal wall, through which surgical instruments and optical devices are inserted. The image from the camera is transmitted to a computer screen with multiple magnification, which allows the surgeon to perform manipulations with maximum precision.

Laparotomy access

A longitudinal incision is made on the peritoneum, about 20 cm long. It is used for large and inconveniently located neoplasms, as well as for gastric perforation and opening of internal bleeding.

Potential complications after gastric resection and their risks

Gastric resection is a rather complicated operation. Some patients may experience bleeding, inflammation of the anastomosis area, peritonitis, indigestion, anemia and some other complications.

Recovery after surgery can last from several weeks to several months. It all depends on the extent of resection, the use of laparoscopic or laparotomic techniques, the presence of concomitant diseases and other individual characteristics of the patient.

As for nutrition, after completing the rehabilitation stages, it is recommended to adhere to the following principles:

  • eat in small portions at least 5 times a day;

  • drink no more than 200 ml of water or other drink at a time;

  • cook dishes by boiling, baking or steaming;

  • eat warm food, not too hot or cold.

The condition of each patient is individual, so after the operation the doctor gives personal instructions. They may differ from the general rules and are more important. How the rehabilitation period will go depends on following these recommendations.
