Treatment of gastrointestinal cancer at the private clinic Oxford Medical Kyiv

Treatment of duodenitis

Duodenitis is an inflammation of the duodenum, a section of the digestive tract that plays a key role in food digestion and in regulating the functioning of the stomach, liver, and pancreas. It is here that the stomach contents mix with digestive juices, and excess acid is neutralized. Therefore, any inflammatory changes in this area quickly affect well-being.

When inflammation of the duodenum develops, pain appears in the upper part of the abdomen, a feeling of heaviness after eating, nausea, and heartburn. The disease may have an acute nature with pronounced symptoms or, in the absence of timely treatment, acquire a chronic course. A timely consultation with a gastroenterologist makes it possible to establish an accurate diagnosis and select effective treatment in order to prevent the development of complications.

Types of duodenitis

Duodenitis is classified according to several criteria: by course, by the nature of morphological changes in the mucous membrane, and by origin. This division makes it possible to determine treatment tactics more precisely and assess the prognosis.

By course:

  • acute — develops suddenly, is accompanied by pronounced symptoms, and requires active therapy;
  • chronic — lasts for a long time, with periods of exacerbation and remission, and is often associated with concomitant diseases of the gastrointestinal tract.

By the nature of mucosal damage:

  • superficial (catarrhal) — with inflammation without deep defects;
  • erosive — with the formation of superficial lesions (erosions);
  • atrophic — with thinning of the mucous membrane and a decrease in its functional activity;
  • hypertrophic — with thickening and structural changes of the mucous membrane.

By origin:

  • primary — occurs as an independent disease;
  • secondary — develops against the background of other pathologies, in particular gastritis, peptic ulcer disease, disorders of bile excretion, or pancreatitis.

Accurate determination of the type of duodenitis is important for selecting an individual treatment regimen and monitoring the course of the disease.

Causes

Duodenitis develops when the mucous membrane of the duodenum is exposed to irritation or aggressive influence for a long time. Most often, the inflammatory process has not one but several causes that act simultaneously.

The main factors include:

  • Helicobacter pylori infection, which damages the mucous membrane and maintains chronic inflammation;
  • increased acidity of gastric juice and reflux of acidic contents into the duodenum;
  • concomitant gastritis or peptic ulcer disease;
  • irregular meals, overeating, excessively spicy, fatty, or fried food;
  • alcohol abuse;
  • long-term use of nonsteroidal anti-inflammatory drugs;
  • stress and sleep schedule disturbances.

Sometimes duodenitis occurs as a consequence of impaired bile outflow or changes in the motility of the gastrointestinal tract. Establishing the specific cause makes it possible to choose the optimal treatment tactics and reduce the risk of recurrent exacerbations.

Duodenitis: symptoms in adults

Symptoms of duodenitis in adults depend on the form of the disease, the level of acidity,
and the depth of damage to the mucous membrane.

Typical manifestations include:

pain in the upper part of the abdomen;

a feeling of heaviness, fullness of the stomach;

nausea, sometimes vomiting;

heartburn, sour belching, bloating.

List of diseases

In a chronic course, the symptoms may be less pronounced, but may periodically intensify during exacerbations. If the discomfort recurs or persists for a long time, it is worth consulting a doctor for examination and clarification of the diagnosis.

Duodenitis: diagnosis of the disease

It is impossible to confirm the diagnosis of duodenitis based on complaints alone, since its manifestations are often similar to gastritis or peptic ulcer disease. Therefore, a comprehensive examination is necessary to determine the cause of the symptoms.

The main diagnostic method is gastroscopy — an endoscopic examination that makes it possible to directly assess the condition of the mucous membrane of the stomach and duodenum, detect inflammation, erosions, or other changes. If necessary, a biopsy is performed during the procedure for morphological analysis.

Additionally, the following may be prescribed:

  • tests for Helicobacter pylori;
  • general and biochemical blood tests;
  • ultrasound of the abdominal organs.

A comprehensive diagnostic approach makes it possible not only to confirm the presence of inflammation, but also to determine its cause, which is key to effective treatment.

Methods of treating duodenitis

Treatment of duodenitis is selected depending on the form of the disease, the cause of the inflammation, and the depth of damage to the mucous membrane.

Drug therapy may include:

  • proton pump inhibitors or H2 blockers — to reduce acidity and lessen the aggressive effect of gastric juice;
  • gastroprotective agents — to protect and restore the mucous membrane;
  • antibacterial therapy — in confirmed Helicobacter pylori infection;
  • antispasmodics — in case of pronounced pain syndrome;
  • prokinetics — in the presence of impaired intestinal motility.

In an acute process, treatment usually lasts several weeks and is aimed at relieving inflammation and symptoms. In the erosive form, treatment is longer, since it is necessary not only to reduce inflammation, but also to ensure healing of defects of the mucous membrane.

In chronic duodenitis, therapy has a staged nature. First, exacerbation treatment is carried out, after which maintenance therapy is prescribed to stabilize the condition of the mucous membrane.

An обязательный component of therapy is correction of lifestyle and nutrition in duodenitis: fractional meals, refusal of alcohol, smoking, excessively spicy and fatty dishes. A comprehensive approach makes it possible to achieve stable remission, reduce the frequency of exacerbations, and prevent the development of complications.

Possible complications of inflammation of the duodenum

If inflammation of the duodenum remains untreated for a long time or frequent exacerbations occur, damage to the mucous membrane gradually worsens. This increases the risk of complications.

The most common complications include:

  • erosions and ulcerative defects of the mucous membrane;
  • gastrointestinal bleeding;
  • cicatricial changes in the duodenum with impairment of its motility;
  • impaired bile outflow due to swelling or inflammation in the area of the opening of the bile duct;
  • development of inflammatory diseases of the biliary tract, in particular cholecystitis (inflammation of the gallbladder);
  • reactive changes in the pancreas.

A prolonged chronic process can negatively affect the quality of digestion, cause unstable bowel movements, decreased appetite, and general weakness. That is why it is important to undergo examination and treatment in a timely manner.

Frequently asked questions

How long is duodenitis treated?

The duration of treatment depends on the form of the disease and the cause of the inflammation. In acute duodenitis, improvement usually occurs within 2–4 weeks provided that the doctor’s recommendations and diet are followed. The chronic form requires longer therapy and monitoring, since it is important not only to reduce the symptoms, but also to stabilize the condition of the mucous membrane and prevent exacerbations.

Why is duodenitis dangerous?

Without treatment, duodenitis can lead to erosions and ulcerative defects of the mucous membrane of the duodenum. In severe cases, bleeding, digestive disorders, and the spread of the inflammatory process to neighboring organs are possible. A prolonged course negatively affects quality of life, causing chronic pain and worsening well-being.

Can chronic duodenitis be cured?

Chronic intestinal duodenitis has a prolonged course with periods of exacerbation and remission. It is not always possible to completely eliminate the chronic process, however, with properly selected treatment and adherence to recommendations, it is possible to achieve stable remission, reduce the frequency of exacerbations, and maintain a stable condition.

Where does duodenitis hurt?

Pain in duodenitis is most often localized in the upper part of the abdomen — in the epigastric area or slightly to the right of the center. It may occur on an empty stomach, 1–2 hours after eating, or at night. The nature of the pain is usually aching, sometimes accompanied by a sensation of burning or heaviness.
